At approximately 0750 eastern standard time on 22 November 2006, Agence metropolitaine de transport commuter train 928, proceeding southward for the Montreal Central Station, Quebec, struck a passenger at the Roxboro-Pierrefonds Station in the Roxboro-Pierrefonds district of Montreal, at Mile 13.40 of Canadian National’s Deux-Montagnes Subdivision. The passenger was struck when she started to cross the main track after reversing direction on the north platform link. The passenger sustained serious injuries.

TSB Railway Investigation Report R09D0053
On 09 September 2009, at approximately 0130 Eastern Daylight Time, while switching at the Montreal Maintenance Centre in Montreal, Quebec, VIA Rail Canada Inc. locomotive 6425 collided with a cut of empty coaches on service track S2W. The locomotive and three coaches sustained damage. There were no injuries.

On 23 August 2010, at 1630 Eastern Daylight Time, 17 cars (16 loads and 1 empty) from Canadian National train M-365-21-23 derailed at Mile 165.80 of the Saint-Maurice Subdivision, near Clova, Quebec. Approximately 1300 feet of track was destroyed. There were no injuries and no permanent damage to the environment.

On 18 August 2010 at approximately 0330 Mountain Daylight Time, CN switching assignment L602-23-17 was shoving 50 loaded and 5 empty cars eastward into track VC-64 at Scotford Yard when the movement collided with a cut of 46 empty cars in the track. As a result of the collision, 43 cars derailed including 21 cars of dangerous goods or residue. There was no release of product and there were no injuries. The cars were intended for track VC-63.

At about 0700 Central Daylight Time on 14 June 2010, Canadian Pacific Railway freight train 290-14 was proceeding southward at 25 mph on the Emerson Subdivision when it was struck by an eastbound garbage truck travelling at a speed of at least 60 km/h, as the train occupied the passive crossing at Mile 13.85, near Grande Pointe, Manitoba. As a result of the collision, 22 car bodies derailed and the fuel tank on the second locomotive was punctured releasing about 4000 gallons of diesel fuel. The garbage truck was destroyed and the driver was seriously injured.

On 04 May 2010, at 0832 Mountain Daylight Time, westbound VIA Rail Canada Inc. passenger train No. 1 struck a 4-door pick-up truck at the Winterburn Road level crossing, Mile 10.76 of the Edson Subdivision. As a result of the collision, the 3 occupants of the truck sustained fatal injuries. The crew and passengers on the train were uninjured.

At approximately 1500 Eastern Daylight Time on 30 March 2010,Canadian National freight train M37631-30 was proceeding eastward from Toronto to Montreal, when 4 of its locomotives and 11 of its cars derailed near the GO Train station at Mile 1.40 of Canadian National’s York Subdivision in Pickering, Ontario. The fuel tank on one of the locomotives was punctured and released approximately 50 litres of diesel fuel which caught fire. The fire was quickly extinguished by the local fire department. There were no dangerous goods involved and no injuries.

On 03 March 2010 at about 1410 Pacific Standard Time, Canadian Pacific Railway Train 300-02 operating eastward on the north track of the Mountain Subdivision approaching KC Junction, British Columbia, side collided with westward Canadian Pacific Railway Train 671-037 that was departing Golden from the north track through the crossovers onto the south track. As a result of the collision, 3 locomotives and 26 cars derailed. The crew on Train 300-02 were transported to hospital for observation. The locomotive engineer was later air-lifted to a Calgary hospital in serious condition.

On 02 March 2010, at approximately 1420 Eastern standard time, Canadian National freight train M36921-02 was proceeding westward at approximately 50 miles per hour when 26 cars derailed at Mile 92.10 of the Kingston Subdivision near Morrisburg, Ontario. Approximately 1400 feet of track, including four crossovers, were damaged. There were no injuries and no dangerous goods were involved.

On 03 February 2010, at 1720 Mountain Standard Time, Canadian Pacific Railway freight train 292-02 was proceeding eastward on track 2 through Medicine Hat Yard in Brooks subdivision, Medicine Hat, Alberta, when 7 empty auto carrier cars derailed. Derailed cars contacted equipment on adjacent tracks to the north and south, derailing 2 locomotives and an additional auto carrier car. Fuel tanks from the 2 derailed locomotives were punctured, releasing 9100 litres of diesel fuel (UN1202). There were no injuries. No other dangerous goods were involved.

On 19 December 2009 at 0223 Central Standard Time, southward Canadian Pacific Railway (CP) freight train 870-013 collided with the tail-end of southward CP freight train 2-298-16 which was stationary on the same tract at Mile 159.31 on the Weybum Subdivision at North Portal, Saskatchewan. Eight residue tank cars (last contained gasoline), UN1203) from freight train 2-298-16 derailed and struck 2 auto rack cars from CP freight train 2-199-15, which was stationary in an adjacent track. One locomotive from train 870-013 and 24 freight cars from the other 2 trains were damaged. There were no injuries. No product was released.
Canadian National Freight Train Q11459-03 Mile 222.90 Spy Hill, Saskatchewan -05 December 2009

On 05 December 2009, at about 0700 Central Standard Time, Canadian National freight train Q11459-03 proceeding eastward on the Rivers Subdivision near Spy Hill, Saskatchewan, derailed 36 cars, including 22 tank cars loaded with dangerous goods, at Mile 222.90. Dangerous goods were subsequently released, resulting in a fire, which initially involved 34 of the cars and burned for 6 days. All residents within a 1.6 kilometre radius were evacuated. Approximately 400 feet of track was destroyed. There were no injuries.

On 21 November 2009, at 2046 Eastern Standard Time, Canadian National train M-365-21-21 derailed 10 cars (5 loaded and 5 empty) on the railway bridge across des Envies River at Mile 6.53 of the Lac-St-Jean Subdivision, near Saint-Tite, Quebec. Approximately 200 feet of track was damaged and one span of the bridge was destroyed. The accident caused no injuries
and no dangerous goods were released.
Canadian National Freight Train J31753-15 Mile 7.9 British Columbia 17 November 2009

At approximately 0324 Pacific standard time on 17 November 2009, Canadian Pacific coproduction freight train J31753-15 was proceeding westward from Boston Bar, British Columbia to Vancouver, British Columbia, when it struck a debris slide of rock, mud and trees that derailed the locomotive and three cars loaded with copper concentrate at Mile 7.9 of Canadian National’s Yale Subdivision nearKomo, British Columbia. The locomotive overturned on its side, injuring the crew. No product was lost from the derailed cars.

On 30 October 2009 at about 2225 Pacific Daylight Time, Canadian Pacific Railway Train 355-429 operating westward on the signalled siding track at Redgrave, British Columbia, on the Mountain Subdivision, side-collided with eastbound Canadian Pacific Railway Train 110-30 that was stopped on the main track. As a result of the collision, two locomotives and six cars derailed. There were no serious injuries. Approximately 3000 gallons of diesel fuel spilled.

On 14 October 2009, at 1514 Pacific Daylight Time, southbound VIA Rail Canada Inc. passenger train 198 struck a vehicle on the Dorman Road level crossing at Mile 75.68 of the Victoria Subdivision. As a result of the collision, two occupants of the vehicle sustained fatal injuries and a third was taken to hospital in serious condition. The crew and passengers of VIA Rail Canada Inc. passenger train 198 were uninjured.

On Sunday, 16 August 2009 at 2005 Eastern Daylight Time, the locomotive diesel engine of VIA Rail Canada Inc. passenger train 46 caught fire while the train was travelling eastward at about 95 mph. A loss of power caused the diesel engine to shut down and the brakes to apply. The train came to a stop at Mile 14.26 of the Smiths Falls Subdivision, near Richmond, Ontario. A total of 323 passengers were evacuated. There were no serious injuries to the passengers; however, one of the locomotive engineers required medical attention for exposure to smoke and fumes.

On 17 July 2009, at approximately 1900 eastern daylight time, Canadian National remotely controlled yard assignment YLUS-30 derailed five tank cars at Limoilou Yard in Québec, Quebec. A turnout and about 180 feet of track were damaged. No one was injured, but there was a minor fuel leak.

On 28 June 2009 at 0631 Central Daylight Time, while proceeding westward on the Redditt Subdivision, Canadian National train Q10131-27 (Train 101) collided with the tail end of Canadian National train M30131-27 (Train 301), which was stopped on the main track at Mile 105.70. As a result of the collision, the four tail-end intermodal cars (six platforms in total) from Train 301 and the three head-end locomotives from Train 101 derailed. The locomotive engineer from Train 101 was transported to hospital with minor injuries.

At 1415, Eastern Daylight Time, on 05 June 2009, westward Canadian Pacific Railway freight train 235-04 derailed 4 head-end locomotives and 27 cars at Mile 174.41 of the Belleville Subdivision in Oshawa, Ontario. The derailed cars included a dangerous goods tank car loaded with hydrogen peroxide (UN 2015), which was not breached. Locomotive fuel tanks were punctured and about 3000 gallons of diesel fuel were released and ignited. Emergency responders extinguished the fire and contained the spilled diesel fuel. As a precaution, 105 nearby homes were evacuated. There were no injuries.

On 03 June 2009, at approximately 0300 Eastern Daylight Time, the Ottawa Valley Railway freight train, the Cobden Turn, travelling westward, derailed two locomotives and seven cars at Mile 60.1 of the North Bay Subdivision near Hodgson, Ontario. The locomotives overturned, resulting in minor injuries to the crew. Approximately 17 000 litres of diesel fuel leaked from the locomotive fuel tanks, most of which flowed into the Ottawa River.

The derailed cars included a load of sodium chlorate (UN 1495) and two loads of propane (UN 1075). The derailed cars struck a wayside propane tank that fuelled a switch heater, igniting a fire and causing an explosion. There were no injuries.

On 14 January 2009, at approximately 0330 Central Standard Time, Canadian National freighttrain M-30451-11 was proceeding eastward at 4 mph when it experienced an undesired emergency brake application and came to a stop at Mile 238.30 of the Redditt Subdivision near Dugald, Manitoba. Subsequent inspection revealed that the train had separated and the A-end stub sill of dangerous goods tank car UTLX 37605, loaded with approximately 51 500 pounds of propylene (UN 1075), had severed and pulled out of the car. There was no release of product, no derailment, no track damage and no injuries.

On 31 December 2008, at 2315 eastern standard time, 33 cars derailed at Mile 46.46 of the Canadian National Drummondville Subdivision, near Villeroy, Quebec. A switch heater was damaged, causing approximately 2000 gallons of propane to be released. About 50 homes were evacuated. Approximately 500 feet of track was destroyed. There were no injuries and no permanent damage to the environment.

On 29 December 2008 at 1448 Pacific Standard Time, the Waneta Turn Assignment of the Kettle Falls International Railway ran uncontrolled down a steep grade near Waneta, British Columbia and collided with a cut of 25 stationary cars. Two locomotives and 12 cars derailed. The locomotive engineer received minor injuries. About 3000 gallons of fuel were spilled. Highway 22A was obstructed by the derailment for two days.
